This policy describes information you provide to Eye Design Optical through our website and SMS program. Our HIPAA Notice of Privacy Practices separately explains how we handle protected health information.
Information You Provide
If you submit a question or service request through our website, we use the information you provide to respond and coordinate the requested services. Providing a phone number, submitting a website form, or booking an appointment does not by itself enroll you in SMS communications.
SMS Communications
You must affirmatively agree to receive SMS messages from Eye Design Optical. We collect consent verbally during an in-person conversation at our practice. Our staff documents your agreement in your patient record.
Providing your mobile phone number without affirmatively agreeing does not constitute consent.
Messages may include appointment reminders, appointment confirmations, scheduling updates, eyewear or contact lens order and pickup updates, office notifications, and account notifications related to your services. This SMS program does not send marketing or promotional messages.
Message frequency may vary. Message and data rates may apply. Consent to receive SMS messages is not a condition of receiving care or services.
Reply STOP to opt out at any time. Reply HELP for assistance, or contact us using the information below.
Mobile Information Sharing
No mobile information will be shared, sold, or provided to third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors that provide support services, such as customer service, is permitted. Text messaging originator opt-in data and consent will not be shared for marketing or promotional purposes or transferred to another business, affiliate, or sender.
